I have a simple question: Why? The entire design behind that document is to make serving files as simple and lightweight as possible. Since you have to store the images on disk, is there some reason not to just use a file-based webserver (lighttpd, apache, etc.) to serve the raw files, rather than using GeoServer as a primary access point?
