I suspect that this has more to do with them not actually being rendered by anything (certainly not osmarender, nor mapnik), and also by them being almost impossible to find during normal map editing (due to their extremely small size compared to what they represent). Italy doesn't even have "Italia" /anywhere/ in it's description.
