I've worked with islands before. It seems that the key points are: 1. make sure that the same tag is used on the inner and outer, in this case natural=water 2. Make sure that the outer is clockwise and inner(s) are anticlockwise
I took the liberty of changing the islands to match their outer so I expect it to render.
