in germany, the first one implies that cyclists are limited to a myx speed of ca. 6km/h, so yes for us it makes a difference. If it's not legally a footway or cycleway, I thus tend to use highway=path foot=designated bicycle=yes highway=path bicycle=designated foot=yes to express if the way is more a footway or a cycleway.
