What you perhaps want is a rule that draw the "most important" cities before it draws any icons or highway shields that might later cause labels to be dropped. This is not so easy however due to the varying definition of "most important", especially in a tiled environment. Perhaps it is possible to do something like "draw n most populous cities, then the rest" or so.
