Certainly not all - in many situations old names are still widely used. In that case I have no clear opinion - I have limited experience with mapping and using names. I would be careful and avoid removing anything. But I would not add old_name that is currently completely unused. BTW, old_name is currently not described properly on wiki - is it even supposed to be used for names that are no longer used?
