The 4th state is when the focus is on the header of the mail. Try it: put the focus there, and press Tab or Shift+Tab. But it is a bug. Is there BT I can support? (Huhh.. I was trying to find a shortcut, that would highlight part of the message header, I pressed Ctrl+Alt+ left arrow...
