I had the same for a while, buggered me quite a bit, until I found out that I had this folder template that had some fancy formatting: %To=""%To='"%OFromName on" <%OReplyTo>'%- The silly thing was that it never got triggered, since I was using AB templates for that list, but then I never tried to PM from a list message.
