Sorry for the delay in responding, but thank you for pointing this out. As you might imagine, there is a process for putting content on our Web sites and during this process, when the document was converted to a PDF, some of the references and formatting were lost. This has been corrected for the new version posted there. Also unfortunate, I do not get notified when the new version actually gets posted to the site so that I may note it here; but I'll try to check it more regularly when I'm expecting the posting to occur.
