Have you tried the LDIF format?  This format is a plain text format.
In my experience, it seems to be more reliable than CSV and other text
delimited formats for transferring information.

When I imported my address book from Netscape, I did it through the
LDIF formats with no problem. Try creating an address book entry or
two in TB with lots of info filled in, and export it to an LDIF file.
This should give you a good example of what TB calls each field.
