This is usually because where, the threads break, they have been
contributed to by folk whose email software doesn't supply
"References" headers.
It has often been theorised in this very forum that TB would benefit
from a "fusion" threading method which combines References and
subjects. Until such time as such a wish can be fulfilled we have to
settle for breakages.
