They use the name defined in the address book in the case where a
"reply" has put in a name which doesn't conform to how you normally
address that individual. The syntax I have used allows these names to
default to the name from the original TO address in the case where
there is no address book entry available.

Watch those folder templates!!!!
Yours is wrongly set up and just has a %TO='blah' without the %to=''
first.
You don't need to use folder templates for this list - use Address
Book templates instead.
