AFAIK, this is intended behaviour. The 'send' toolbar button always
sent messages immediately despite any other settings you may use.
Not very friendly for those who like using the mouse but this is how
it has been since as far as I can remember. It does however give you
the means to bypass your confirmation or 'queue in outbox' setup so
that a message may be sent immediately without changing
configuration.
