Most ISPs actually offer multiple accounts, just neglect to tell you, and if they don't offer them for free, they normally charge (as you said) a small fee. My old ISP in the UK allowed me to setup up to 5 mail boxes, each mail box could have up to 3 aliases assigned to it. There are other ISPs in the UK that don't provide a specific address, but give you a subdomain (virtual anyway) so you can setup whatever you want.
