I have but the symptoms only showed up with the default mailto: linked
to a password account.

Did you by any chance install The Bat! as the simple MAPI client?

On this system (Win98SE) having the bat mapi.dll installed meant the
link was being send both by DDE (shortcut settings in registry) AND by
MAPI (which would try and open a new Bat session (thus the second
login prompt).  So I replaced the old mapi.dll and things worked
again.
