Maybe I'm missing something, but it seems the simplest solution would be
to just add a line to the beginning of all emails you send now saying
"This email address will be effective only until December 19th. My email
address after that time will be ***@tiscali.co.uk". Is there some reason
that won't work?

Other thoughts:

Re the problems sending using your current smtp - why would the error
message come from tiscali if you're using the smtp for your current
smtp? Surely the error message would come from your own ISP? I'm not a
techie, so I don't know, but it seems odd.

Other UK ISPs do allow you to send using a different smtp server - it
might be worth your while opening an account with one of those. You can
open an account with most UK ISPs where you pay only for your local call
when you connect to that ISP - ie, you can use the email from that ISP
without any charge. Marck suggested free-online.co.uk to me & I find it
pretty good (though I do know someone who had problems over payment for
their flat-rate account with them, so I'd stick to the pay-as-you-go
option). There are lots of others which do the same. Orchidserve.com is
very fast, & again, free. Or you could use a web-based email address
which allows pop access, such as eudoramail.
