I use SpamAssassin (yes, in Windows). It's free and pretty powerful. Very configurable. Not all that hard to set up, but requires Perl and Pop3Proxy (both free aswell). I installed PopFile for a friend of mine. It's easy to set up and to train, so that seemed the best solution for her. It's free aswell.
