Have you checked the error messages in the log? I see this from time to time when a connection gets killed for whatever reason, and you attempt to connect before the lock file on the server is removed. The server returns an error message about not being able to log in because of the lock file, but TB sometimes prompts for the user/pass combination again. As they are valid, by the time you click OK, the lock has been removed because they tend to have an expire time.
