Instead of closing from there, click once on the inactive TB! window
(so that Windows knows which program you mean) and hit crtl-F4 once.

Let us know whether TB! suddenly becomes focussed.

On occasions, what you describe happens over here too (single
monitor), but only upon startup. I'm using 3.99.3 but it has happened
for several versions. Only very few people can confirm it.
