I like Zoomerang.com but you can only get the data for 10 days after 
launching with the free version.  I'm not sure if the free version 
allows downloads into SPSS, either.  Something to check.  It's what I 
use to do mid-course evals since it's anonymous, etc.

I'm also getting ready to try Survey Monkey.  I hear good things.
