Josh,

If you are running a Joomla there are a few extensions that do a great job of 
building and collecting form info.  I am using JForms (Price $0 MD) and it lets 
you build fairly complexed forms with a WYSIWYG interface and then it can mail 
out the results, AND keep the info in a table for you to retreive later.  Works 
well for me.

If you were thinking about droppin in a Joomla this might be another reason to 
consider it.
