Actually there were a couple of web based services that would set up a call
w/ a spoofed number and connect it to your phone and your "victim". Don't
know if it is still done?

phone system hands it with no verification.   A while back a
misconfiguration in our phone system was causing the Caller ID numbers to
appear as if they originated from one of the local hospitals.  A 3rd party
or internet-based phone company could very easily do this, perhaps even
provide it as a service so that when a call is placed, it randomly hands out
number or, legitimately routes the call randomly through any of its assigned
number pool.
