I haven't read this whole thread, so I apologize if this has been discussed before, but... do you by chance have a mail filtering appliance? I've run into something like this before, and the problem was that I've only authorized certain internal IPs to our spam filter appliance that allow relaying mail through the same. This was put in a place a few years ago when we were notified by ICN that a small amount of spam seemed to be coming through our IPs into their system. So I was forced to lock down our internal network to just a few specifically-allowed SMTP relays.
