sounds like a workshop registration type app??
I built this at my last job, then modified it for my current school.

There's a MySQL db that stores all the data. The admin (Curriculum
Director in my case) has a Microsoft Access front-end that allows her to
create the workshop, set various options, generate sign-in sheets and so
on. The original app actually launched a Word Macro to build a
mail-merge document to generate the CPDU forms with names pre-populated.
There is also a web interface to view all registrants of a workshop.
It doesn't do any calendaring, though.
