We used Radiator at Belgacom Skynet (the largest ISP in Belgium), to serve up to one million residential & business customers. Rock solid -- and open source. We did have some issues with the UDP packets getting dropped due to congestion, so what we did was to put multiple RADIUS servers out there co-located with the equipment which terminated the phone and DSL connections (effectively acting as local RADIUS proxies), and then those RADIUS servers would connect over TCP to our central database servers.
