The `man Template::Manual::Config` man page implies that setting 
"CACHE_SIZE" to zero should turn off caching.

Have you tried stopping and starting the mod_perl server? Not just 
getting it to reconfig but truly stopping it? If you aren't actually 
stopping the server entirely then I'm pretty sure that TT won't be 
called to throw a new instance with new settings from just a soft restart.

I'd also follow the advice of clearing your cache directory if you once 
had it enabled...
