Since everything comes out alright in your example for me, I suspect
that some plain routines like \makeheadline and \makefootline are
missing. Could it be that you use a musixtex format? I would discourage
to do that and use instead the plain format and load the modules that
are necessary, including musixtex. Did you check the log-files of the
TeXing?
