For a non-indexed slur, this should never be the case. I don't think it ever
was the case but I can't be sure.

True before Ap, but when I introduced postscript slurs and ties, I also made
a real distinctions between slurs and ties.

Rather than try to make a list of rules, here's a sample file with each line
followed by a comment indicating whether the MIDI file is tied or not.
Basically, only non-indexed s-slurs, and non-indexed postscript t-ties lead
to midi ties. If you use any version before 2.5 and get a different result,
please update. If you choose to update and get a different result, then
please get back to me.
