As much as I like a PMX challenge, I can't see how to get BOTH the desired
notation and MIDI output with a single PMX file alone without in-line TeX. I
bet Andre can see how to get the right MIDI output but with the wrong
notation; and Christian's abcpp suggestion could then give the desired
results without any in-line TeX.

For the MIDI files on my web site (www.geocities.com/pchpublish) I always
ended up having to make a special PMX input file to get things just right.
