Take a look at my PMX  tutorial, Sec. E 1.3 ``Transposing
instruments'' (pp.94). It should anwer your question fully (but
observe the note on p.96!).

You might also want to take a look at Sec. B 5.5
``Transpositions'' (pp.56).
