Comments:

1. In your source you shold explicitly set the octave (by number) at the
start of every input block for each voice.

2. I don't understand where your errors came from, because I cannot
reproduce them. It compiles on the first try. Still, the octaves are all
messed up.

3. You can email sources directly to me instead of the list if they're too
long to post. The one you posted isn't too long.
