Q1         Can physical examinationsgive doctors valuable information that
cannot be obtained by whole-body MRI?

A1          Tiny Nair Cardiologist at PRS Hospital, Trivandrum11h ago

The traditional fisherman, stands at the seashore, looks out at the
sea, carefully watching the waves. He can pretty accurately predict if there is
a storm brewing, or itsgoing to be a laid-back trip with a lot of catch. When
in doubt, hesupplements his sixth-sense with the local ‘weather-report’ before
he embarkson his boat.

MRI is an extremely sensitive imaging system that can detect
minisculestructural abnormalities, with a resolution far sharper than any other
scan like CTor ultrasound. It can also identify the nature of the tissue – fat,
fiber, muscle, as well as amount of blood percolating through the organ
(perfusion). That’s why it is one of the most sensitive imaging systems.

A CT scan of a ‘just-dead’ person may be normal but not the MRI. CardiacMRI
(CMR) can confirm diagnosis of complex heart disease that in the past couldonly
be proven by cardiac biopsy.

But it is still a scan.

Consider lifestyle diseases – hypertension, diabetes, dyslipidemia
(highcholesterol); infections in the blood – typhoid, malaria, dengue;
functionaldisease like bronchial asthma, or metabolic disease like
electrolytedisturbances; all these may have a perfectly normal whole-body MRI
scan.

A patient presenting in an unresponsiveness to the ER, suspected to havesome
complex stroke, sent for MRI, which turned out to be normal. Blood
reportsshowed severe hyponatremia (low sodium level). Corrected, he walks back
home in3 days.

The physician holding the patients pulse feels the waves that reflect
thehemodynamics at the ventricle in the heart. The rate, rhythm, volume gives
thehim an overall idea of the storm that might be brewing in your
cardiovascularsystem. Listening to the story of your illness, using his senses
to see, feel, palpate, and by using few simple tools like the stethoscope, and
ECG, thedoctor makes possible diagnostic scenario, and then narrows them down
orderingtests including MRI or angiogram if necessary. A high clinical
pretest-probability tend to point to ahigher likelihood of an MRI scan picking
up the right diagnosis. Enormous costsaving is just part of the benefit.

Compared to the 1.5 million-dollar MRI scan machine, the physician’sclinical
examination, akin to the fisherman staring to study the waves, lackssomething.
Glamour.

Q2         As a landlord, who(without naming names) is a tenant you will never
forget, and why?

A2          Ch. Lavanya Lecturer May17

A married couple with teenage children came for renting the house. We
immediately gave them as theylooked family type.

Later after few months we started hearing quarrels at night time. Theirson was
with wife's sister as she was a divorcee and daughter stayed with them.

Husband used to work in a small factory and was very calm going man. Heused to
leave at 7:30 in the morning and come back at 8 in the evening. Wife was over
cleanliness lady and was always seen washing her handsunder the tap.

One day she came to us shouting “ He(vulgar word) took away my Mangalsutra
while I was asleep. How dare he….” going on ranting and abusing him.

Again another day she came with a weird complain “ We had fight at nightand he
pulled my hair (another new vulgar word). I'll drag him to court..”

When he was away she used to fight with her daughter and showed mescratches by
her daughter's nails on her hand.

This went on for few years andone fine day packers and movers came and took
away all the furniture.

They both parted away forever. She took both children with her and he wasalone.

Later I heard he took a room for rent nearby his company and lived allalone and
she stayed with her sister and mother.

She was called Miss cleanliness! I never understood who was at fault.

Q3         What is the mostinteresting fact that you know and I don't, but I
should?

A3          Ayan Srivastava B.E fromThapar Institute of Engineering and
Technology (Graduated 2019)Updated Wed

So today, the ceiling fan in my room (Let’s call him John 😂) stopped working
suddenly. Since we had a spare, Ialong with dad decided to dismantle it and
install a new one. John had plasticblades.

When I climbed the ladder to reach up to him, I noticed somethingatypical. John
was hella dirty (pun intended), but for some reason most of the dust seemed to
accumulateon one edge of the plastic blades (the one that faces air directly,as
shown in blue). I remembered seeing this a gazillion times before also (itused
to make cleaning the fan blades so easy! Just a single wipe n you’re done)

But why? The blades move so fast! Why catch dust at all? Even if dust is caught
somehow, why isn’t it shaken off?And what is this weird partiality with one
edge specifically? What is going on!

To understand what’s and why’s , let’s break it down. The fact that dustis
collected, how much and where, is depended on a few things:

1. Dust present in air.

2. The fact that dust and blades can have a charge in the first place.

3. Some weird thing called boundary layer.

A) Dust in the air:

Air around us, in general, carries a LOT of just. Like this:

Naturally, more the dust, morethe probability of the blade catching it and more
dusty it becomes (explained below).

B) Charges (Static electricity)

Remember we used to do thatexperiment, taking a comb, rubbing it in our hair,
and attracting pieces ofpaper. It used to happenbecause the comb picked up
electric charge and attracted (by induction) paperpieces.

Dust is also electrically charged. Usually the dust you find stuck to plastic,
TV screen etc. will havesome sort of +ve or -ve charge. If a positively charged
dust particle comesinto contact with a surface having negative charges, it will
stick to it. Thisis due to static electricity.

You may also have guessedcorrectly that dust particles can in fact, stick to
one another if they haveopposite charges.

Let’s talk about the issue now : AIR and BLADES

Air is a fluid (a fluid is anything that can flow). It is made up ofdifferent
gases having molecules. When the blades rotate at a high speed, it is cutting
through the airand knocks gas molecules around. Electrons are knocked around
and anet charge gets built up on the blade.

Now you got it didn’t you 😃

Those dust particles in airthat have negative charges will get stuck to
positively charged areas of theplastic fan blade, and vice versa. The fan speed
doesn’t matter here as the static electricity force isquite strong when
particles are small. Blades will always dust up eventually.

In fact when fan moves faster, it will displace more air (with so muchdust) and
there is a higher chance of dust attaching to blades.

Also, it deposits more on leading edge (the one facing air first) becausemost
of the “hitting air molecules” starts from there, and most charge is builtup
there. So max dust attraction. Also dust+dust also attracts, so there’sthat.

C) The Final Culprit: Boundarylayer (B.L)

Devil is in the details. All you learned above is NOT true for metalblades.

Metal blades (if given in a fan and they are grounded) will hold nocharge. So
the dust will never deposit (can’t stick to anything) and it willfall off.

Except dust still sticks 🤣🤣🤣(Persistent bastard!)

Let us understand what B.L is. It’s a simple thing.

(Ignore the words: Laminar, transition, turbulent, LBL, TBL)

Now, let us say Mr Air moves with a speed of 1 meter/sec. As shown by thepurple
“equal length” arrows at far left, this tells that before the air hitsblade
surface, all particles have EQUAL velocity(direction is same &magnitude
indicated by arrow length,also same)

As air moves past the blade, particles at the bottom “stick” to surface. There
is NO velocity at blue dot, hence no arrow also.

As we move upwards from fan surface, air starts to gain ‘some’ velocity
(lengthof arrows is increasing) and finally attains full velocity at some
distance.Entire red shaded area is BL(from 0 to max velocity)

This is why dust sticks to metalblades too! Since very nearto surface of blade
(where the ‘sticking’ happens) has air flowing with verylow velocity, it is not
enough to blow away that dust(which is strongly held bystatic electricity).

This B.L reason can be appliedto plastic blades too.

So that’s it. Using science onecan appreciate even dust 😂😂

EDIT 1: Since John was dismantled, RIP John. You taught us a lot😂

EDIT 2 : If someone is having confusion between plastic and metal blades, google
“Van de Graaff” generator. It gives an example of metal sphere so muchinsulated
that charge never leaves. But metal blades may be grounded so staticcharge
won’t remain there.

Q4         South Indians want onlyEnglish as common language in India, because,
while they don't know Hindi, theyare familiar with English, due to their school
and college education in Englishand it's usefulness in employments. Are they
not reasonable?

A4          Bala Senthil Kumar KnowsHindi May 18

South Indians just want to be left alone to make choices on what theywant to
learn or use. If Hindi is going to make anybody’s life better, there’sthe
opposite proof of standard of living in the Hindi belt of India, comparedto
South India.

South Indians score pretty highon the Hindi Prachar Sabha exams, which means ‘
दक्षिणभरथियों को हिंदी सीखने में कोई दिक्कत नहीं है l “

We just don’t want to have anything imposed on us. We’re even happy tolearn
Hindi if needed by us, not by others!

Q7         What is the saddest truthabout life?

A7          Aakarsh RauniyarMarketerat eSewa - Nepal First Online Payment
Gateway (2021–present)Updated May 17

God created the donkey and saidto him, “You'll be a donkeyand carry everyone's
burden and will live for 40 years.” The donkey said,“Carrying burdens is OK,
but 40 years is too much for me, give me only 30years.” God granted his wish.

God created the dog and said tohim, “You will be a dog andlook after people's
belongings. You'll eat whatever you are given by your mastersand live for 40
years .” The dog said, ”All conditions are fine, but 40 yearsis too much for
this job. Give me only 20 years.” God granted his wish.

God then created the monkey andsaid to him, ”You will be amonkey and entertain
others with your tricks. You will live for 40 years.” Themonkey said, “God 40
years is too much for me, give me only 30 years.” Godgranted his wish.

God then created man and saidto him, “You will be a man and live for 25
years. You will dominate all other creatures.”

Then the man said, ”25 years is too little for a great creature like me.Give me
the 10 years that the donkey refused, the 20 years that the dogrefused, and the
10 years that the monkey refused.” God granted his wish.

>From then on, aman lives 25 years as a man, 10 years as a donkey, carrying all
>the burdens ofhis house, 20 years as a dog, taking care of his children and
>eating whatever he is given, and 10 years as a monkey, goingfrom house to
>house entertaining his grandchildren!

What is the saddest truth of life? Most people never get to experiencewhat's
the difference between living and existing. Most peopleonly exist, don't live.

Q8         What was the biggestculture shock you’ve experienced?

A8          K.Sathia Murthi FormerWarrant Officer at Indian Army Thu

I am from south India (Chennai) but I was born in Kolkata and raised inseveral
north Indian cities (Delhi, Punjab, Rajasthan,)

I had done schooling in almost different schools and colleges.

Since my birth, I was exposed to north Indian culture only but my fathergot
transferred to Chennai and we had to move in there.

Here comes the culture shock, despite the fact I am a south Indian:

Almost everyone (women) has a bindi on their fore head, be it a child ora
lady.(Edit: not all wear it neither is it compulsory but I have seen
mostlyeverybody wearing it.)

People here celebrate the puberty of girls.

Weddings here are performed inthe morning. But in north India, we know weddings
are held at night.

Girls in class 6th and 8th aremore mature (mentally) than those girls in north
India.

And when I came here I wastreated like a baby. Kinda embarrassing. Also, some
bullied cuz of my bad Tamilaccent.

Almost every woman here has mogra flowers on their hair.

If you wear tops and pants people will stare at you until you turn the
nextstreet. (Not all but mostly, nowadays it has been reduced to some extent.)

One day we went shopping and we took a coffee break, the coffee waleuncle gave
us our coffee in a tumbler, I didn't like it bcuz my only thought was how many
peoplewould have drunk from this tumbler even though it would have been
washed. So I didn't drink. But that uncle shifted the coffee to a paper cup and
then Itook. (He was kind:)

Everybody used to call girls‘maa'. For ex: ‘maa, inga van Galen!’Translate:
‘maa, one second come here!’. Idk why but I found it cute cuz theyrespect every
girl as their mother.

Another is ‘RAIN', yes duringwinters in the north, people inthe south
experience the rainy season! They are unpredictable.

I will update more as I recollect.

(Edit: I know behind every point that I have mentioned here, there is
abackground or a scientific reason. But I have answered precisely to the
extentit should be answered without stretching it and straight to the point.
Otherwise,I will have to explain every detail which is not necessary and not
relevanttoo.)

(Edit 2: I am sathia murthi and have written this answer from myexperience and
my daughter's experience)
