Q1         Willdoctors prescribe testosterone?

Yes, doctors prescribe testosterone to those who are deficient and are inneed 
of it. These are mostly aging men in their 60’s or older who no longerproduce 
testosterone. 

A blood test is ordered. If the testosterone is low, testosteronereplacement 
can be ordered either by testosterone-gel or by testosteroneinjection. 

A man who is on testosterone replacement therapy should have follow-upblood 
tests every three months. 

The hormone level is kept inthe high-normal level.

The doctor cannot prescribe to younger men who have normal 
testosteroneproduction, just because they want to perform better in sports or 
they hope toget more powerful ejections. 

It is a myth that this wouldwork, because there is a hormone feedback between 
the circulating testosteroneand the pituitary gland and the hypothalamus. 

Blood testosterone tests in these cases are in the high normal range 
oftestosterone. It is illegal for the doctor to prescribe testosterone in 
suchcases. The doctor would risk losing his/her licence to practice medicine.

The hormone feedback cycleinvolves FSH and LH and the hypothalamic releasing 
hormones. When you overdose on testosterone, the feedback circuitsshut down 
your own production of FSH and LH, which can result in testicularatrophy and 
lack of sperm production. That cannot be good for the younger manwho may want 
to start a family.

Airlock or air binding iscaused by air being trapped in a system. The only 
remedy is to get rid of the air. Find a way to vent the air or to pre-fillthe 
system. 

Possibly by disconnecting a fitting at the highest point in the systemand 
allowing water to flow out before reconnecting the fitting.

My husband was once given his boss's credit card to take me out to a nicedinner

Firstly the water which you are drinking is from the reverse osmosistechnology, 
in the reverse osmosis it removes salt, bacteria, and anotherimpurities from 
the water, where the water which is purified is so purifiedthat it contains 
nothing an but a water after the reverse osmosis there is aprocess of UV 
purification this kills bacteria, germs, viruses, present inwater, again There 
is an additional process where they add minerals to thewater, nowadays 
technologies.

Because by the process of reverse osmosis there is low minerals in thewater so 
the water is used add extra minerals in water must be in condition todrink. 

By the process of UV purificationwhere do we light present in the chamber kills 
the germs, bacterias, in thewater which makes water little warmer too

So you get a warmer water

Q6         Whatis the maximum official stoppage or halt time in Indian trains? 
Some trainshave stoppage more than 20 minutes on big stations.

There are two types of stoppages considered in Indian Railways:

Commercial Hault- A haut fromwhere passengers canboard/deboard and tickets are 
sold.

Technical Hault - A hault wheretrain stops for loco reversal,staff change, 
water fillup etc. Such stops are not commercial and no ticketsare sold for 
these stops.

Stoppage time at commercial hault depends on the type of station. It starts 
from as low as 1minutes to as high as 30 mins. ( scheduled)

If a train has scheduled hault of 20 minutes or more then there isdefinitely a 
loco reversal or attachment/detachment of banker locomotives.

Exceptions: A train can stop for more than 30 minutes in case ofuncleared line, 
technical or mechanical failure.

Technical haults are usually of10–30 minutes in whichloco might get reversed, 
staff might get changed, water is filled in coachesand cleaning is done in 
coaches. You can not buy tickets for/from a technicalhault.

But that’s not what we dread.

Invariably, the last patient would have been one who got an appointmentby 
haggling with my secretary or the ‘sit in’ category, who would have come 
andannounced that ‘I would meet the doctor and go; come what may’ type.

Both often turn out to be suffering from major trouble.

Over the years, the ‘lastpatients’ (at home and in hospital OPD) have had much 
more heart attacks, cardiac arrhythmias or evolving stroke; where thepatient 
believed that he has gas, dyspepsia or a vertigo.

In hospital, it is called the‘last token syndrome’.

Once I see him, with a major issue at my home office at 9 PM, what do Ido? Ask 
him to go to the hospital ER for an ECG and necessary treatment.

How can we avoid this problem?

Simple. 

Why not straight go to the ER? The answer is not so simple. It is not 
thepatients fault.

In the Indian medical system,the doctor is always given an iconic stature; 
while the system is neglected. There are many excellent doctors butthey have to 
work in a rusted system.

In contrast, in the US, the system is very strong. In a strong system anaverage 
doctor does well, while the reverse is not true.

The system

An Indian doctor might wrongly diagnose an atypical chest pain asmuscular and 
miss out a heart attack. In the US, all such pains wouldmandatorily have an ECG 
done. If the ECG shown a heart attack, andunfortunately you are in a ‘Indian’ 
rural hospital, you have to be content withsome aspirin and wait for the next 
bus to take you to the nearest town, whilein the US, the system will make sure 
that you are flown to a higher center in ahelicopter.

It is this lack of belief in asystem that brings this ‘last patient’ to the OPD.

The Ego

‘Don’t do anything without consulting me’

The patient, overwhelmed by the weight of my degrees would just do that,unless 
I tell him that in an emergency don’t wait for me and go to the ER. Thatcan 
happen if the doctor can ensure that the hospital ER is a strong system,capable 
of responding to a crisis. Mostly it doesn’t.

The ‘last patient’ has justobeyed his doctors advise.-

The ‘Over’ Reaction

Just before COVID struck (Home consult is closed post COVID) I saw a 
lastpatient coming in with a huge plaster in his hand.

Time 9 PM

‘Doctor, I was waiting for 2hours to see you’

He sat down.

‘See Mr, I am a cardiologist. This is clearly an ortho problem. Why don’tyou go 
to the right doctor? I am sorry but I am helpless’

He listened to me quietly.After my long lecture ended, he looked at me,and 
smiled.

‘My wife is your patient. Ijust came to ask if she can have a dental 
extraction’.

‘And this is an accident that happened a month back, I am fine and activenow’

As I told you, last patientsalways spring a surprise. Didn’t I?
