hospital case sheets belonging to the unit I wasposted in to check if they were 
complete before storing them for futureretrieval if they should be needed. A 
senior surgeon who was watching me at thetask asked me to check the result 
column. In that column one must make a one word note if thepatient was ‘cured’ 
or just ‘relieved’ or is ‘dead’.

‘Of the three whatwould you choose for a case of inguinal hernia,’ he asked.

‘Cured,’ I said promptly.

‘How do you knowthat it was cured,’ he said.

‘The patient camewith a hernia and at the time of discharge he had no hernia.’

‘Are you sure therewould be no recurrence?’I had no answer.

One day I washaving a haircut. Thebarber during the course of clipping my hair 
told me that a couple of years agoI had operated on him for hydrocele.

His haemaglobin level must have been very lowindeed for he was as pale as

I recognized him.He had a large benign gastric ulcer for which we did a partial 
gastrectomy.

‘Thanks to you the stomach pain isgone,’ he said, ‘but I am unable to eat more 
than a few mouthfuls of rice. Ihave become very weak.

I got himreadmitted and transfused a couple of bottles of blood. Before 
discharging himI arranged for a regular free supply of proteins supplements. My 
operation as far as hisgastric ulcer was concerned was a success but 
nevertheless it was a failure.Our follow-up was inadequate. We had failed him. 
Having removed half ofthe stomach of a rural rice eater we should have done 
more than just hand overa prescription for the protein supplements at the time 
of discharge. The caseneeded more active follow-up. He did not get it.

The case of anaemia was referred from the medical ward.In their note they said 
there was no medical cause for the anaemia but thepatient was complaining of 
piles and loss of blood in the motion.Onendoscopic examination we could see 
three large pile masses. He did have third degree piles.After preparing the 
patient with a transfusion we operated on him and removedthe pile masses. 
Unlike the three previous cases in the case sheet ofthis patient we could enter 
‘cured’in the result column.

Please note thatthe examples chosen are all common surgical conditions. There 
is no heart,lung, or brain disease and no case of cancer either. Even in such 
commonnon-life-threatening cases one cannot confidently enter ‘cured’ in the 
result column. For mostsurgical procedures success is a relative term. Now you 
can see why surgeons never talk about thesuccess of their operations.

As forcomplication they never talk about it for it is bad policy to frighten 
thepatient
