Just a total shot in the dark but I had something similar when I copied and pasted from a text editor into therion editor. The therion editor. I ended up doubling the first line the text encoding line (UTF blahblahblah) which was not entirely obvious because the therion editor seems to hide that line. Once I fixed it the problems went away.
