Honestly - acts_as_ferret and active_merchant. I've been bitten twice
with acts_as_ferret interdependencies that required a different mix of
versions in production (interplay between ferret, index formats,
acts_as_ferret and legacy versions of will_paginate). With
active_merchant, I've been forced to keep a vendorized plug-in for
production because the gem fails to load properly though in
development and on stage the gem works just fine. I freely acknowledge
this is not likely the norm! I've inherited this stack after 4 years
of development work from a very wide variety of contractors, some,
much less skilled then others. I've been burned a couple of times and
now wait to see everything working properly on production before the
version lock occurs.
