Ok, I was able to figure out what the problem is, but I don't know why
the problem occurs.

In my environment.rb I had something like (trimmed):

config.plugins = [ :exception_notification ]

When I uncommented this line, everything worked fine (I did limited
testing), but when this line is present, I get the error I previously
mentioned. I was able to repro this bug with a simple rails app (1
model), and the exception_notification plugin

Any ideas?
