I am having same problem in my prototype server, on my local it works
fine. But on server, it doesnt work until I change include() to readfile()
Concern for the dynamic CSS and JS should be on second priority, or I
guess we need some other workaround in case of dynamic assets. The proper
way to output files with PHP is readfile() only.
