#6260: FormHelper : hidden radio name when passing a name parameter

 When passing a name parameter to the radio method of FormHelper, the
 automatically hidden field generated doesn't have the good name : it takes
 the auto-generated name from $fieldName var.

 Just have to add the name parameter when you call the hidden() method.

 Diff and unit test joined (real world tested).
