All "real" databases run counters for their ID fields. Fields that we think of as identifying (e.g. name, social security, address) often need to be changed. So yes, an IT professional database specialist had better know about counters.
Are you talking about running a TW using mySQL to populate it? I don't know if anyone has done that ... you could check the vault. That would be a fun project, but I don't know how productive it would be. It would essentially change the portable nature of TW. I imagine that there are other Wikis out there designed from the ground up to use a mySql or some other database.
