[[Johny Smith|Smith, John]] should work. Another option is to write in [[Johny Smith]] <<tiddler [[Smith, John]] >>. This will transclude the [[Smith, John]] content into [[Johny Smith]]. You'll find TiddlerWithEditPlugin useful.

The result is what ever you label as a redirect to [[Smith, John]] will automatically become a link to that tiddler.
