Did you use this syntax:
   [img[tooltip|path/to/filename.jpg][path/to/filename.jpg]]

The "tooltip|" part is optional.
The first path/to/filename.jpg is the image to embed in the tiddler
content
The second path/to/filename.jpg is the file to link to

Clicking the image should open the specified file in a new browser tab
