Hi Dadoo¨ Have a look at Tobias' tbsGTD TiddlyWiki... You can create reminders directly from the calendar in his setup.. http://tbgtd.tiddlyspot.com/#desk
You'll find the same functionality in the d3 tw-variant you can choose to create on tiddlyspot.com. Maybe it's not a rewrite of the calendarplugin you need - but an option to create tiddlers with predefined tags, which invoke custom edittemplates. It should be fairly easy to do that - and you don't even need the CalendarPlugin. Just add a <<newJournal "YYY MMM DD" "journal">> button and make a custom journalEditTemplate with the fields you want. Of course you'll need http://www.tiddlytools.com/#TaggedTemplateTweak to make it work... If you use custom edittemplates you'll need to save the tiddler and reopen it to make the custom edittemplate active.. However the remindermacro in Tobias' TW (and d3) is written directly into the new tiddler and you can edit the macro right away.
