I can see the two text boxes(username and password) in mediawiki
unplugged but I don't see it in tiddlyspot, i.e. the plugin I added.
Where should I specify the username in the code?

No, it's only available on our intranet. It's a bit old though,
MediaWiki 1.9.0 - Could this be cause of MPTW maybe?

Even pasting some wiki code gives me the following error in an alert
box:

TypeError: this.source is null
