With help and guidance from Eric I created a simple database in tiddlywiki for storing students' class records last summer.
http://classrecords.tiddlyspot.com/ It is based upon the unamesa style form (also by Eric), in which a tiddler shows an input form in edit mode and the data entered in the view mode. Each record is a tiddler of its own, and some tiddlers have inline script for generating customised lists. You can get ideas from this application.
