Qalisto I am confident a solution can be built on TiddlyWiki for this, I would either host it somewhere on the internet with an appropriate save mechanism eg; tw-receiver (if you must use chrome/FF), or install your wiki inside one of the apps for running tiddlywikis.
I do not think much stripping down is needed, only ensuring you have mobile a handling support/theme.
