I have used $:/plugins/tiddlywiki/comments which is an official TW plugin and I am happy with it. You can install it directly from $:/ControlPanel. For multi-user situations I had used the Disqus <https://tiddlywiki.com/#Disqus%20comments%20plugin%20by%20bimlas> plugin by Bimlas and it had worked very well. (A Disqus account is needed).
