I've had the same problem a number of times. In my case it was caused by a slow internetconnection and an "old" cached version of my TW, which just wouldn't be purged from cache by itself. I don't remember if the problem was mostly occuring in Chrome or Firefox, however a browserupdate, which empties the cache should load the latest version - if it doesn't, you can keep on working on the .../index.html version and be confident (hope) that it will show if you open it in another browser...
