@Eric Schulman I seem to remember that you used regex and TiddlerTweaker for mass changes. What would these be? Perhaps it wouldn't be too tough to wrap that in a RenameTiddlersPlus kind of plugin hijacking the cores' renaming function as MGSDs RenameTags does?
Perhaps later a step based process could be created on top where first the user would be given a list of tiddlers along with a pre-set checkbox, title and the content surrounding the match... and eventually a button to update the selected tiddlers.
