When save of a new tiddler occurs it already checks if a tiddler name exists, so when that "search" is kicked off would be the perfect time to add your code. The problem would be capturing the existing tiddler name before the replacement happens so that it can be used for the update. Seems pretty feasable to me, but I am no javascript dev :)
